General Psychiatric Evaluations

We provide adult psychiatric evaluations and medication management for a variety of mental health conditions, such as ADHD, anxiety, depression, bipolar disorder, OCD, PTSD, and schizophrenia.

What it is

A psychiatric evaluation is a visit where a provider learns about your symptoms and health history. It can help find a diagnosis and shape a treatment plan you build together.(Source 1) If medication is part of that plan, follow-up visits check how it is working.(Source 3)

Who it may help

  • Adults with symptoms of ADHD, anxiety, depression, bipolar disorder, OCD, PTSD or schizophrenia
  • Adults unsure what is behind changes in mood, sleep or focus
  • Adults taking a mental health medication who want ongoing care
  • Adults who want a fresh look at a past diagnosis

What to expect at I Care Psych

  • Visits are in the office or by telehealth.
  • Your provider asks about your mood, sleep, energy, thoughts, alcohol or drug use, and family history. You also fill out an intake questionnaire before or during your first visit.(Source 2)
  • Your provider may suggest lab tests to check for physical causes, such as thyroid problems, when needed.(Sources 1 and 2)
  • At follow-ups, you review how you feel and any side effects. Finding the right medication can take a few tries.(Source 3)
  • Please bring a photo ID and your insurance card, and check your coverage with the office.

Good to know

  • Bring a list of every medication you take, including over-the-counter drugs, vitamins and supplements.(Source 4)
  • Don't stop a prescribed medication on your own, even if you feel better. Talk with your provider first.(Source 3)
  • In a crisis, call or text 988. In an emergency, call 911 or go to the nearest emergency room.(Source 5)

Medication-Assisted Treatment

We provide medication-assisted treatment (MAT) for alcohol use disorder (AUD) and opioid use disorder (OUD) with FDA-approved medications, such as disulfiram, acamprosate, and naltrexone for AUD, and Suboxone and extended-release naltrexone (Vivitrol) for OUD. Off-label treatments for cannabis use disorder, stimulant use disorder, and other illicit substance use disorders are also available.

What it is

Medication assisted treatment (MAT) combines FDA-approved medications with counseling and behavioral therapies to treat the whole person.(Source 1) These medications can ease withdrawal and cravings.(Sources 1 and 4) Some, like naltrexone, block the effects of opioids.(Source 5)

Who it may help

  • Adults living with alcohol use disorder (AUD) or opioid use disorder (OUD)
  • People who want to stop or cut back on drinking: naltrexone helps reduce the urge to drink, acamprosate can make it easier to stay alcohol-free, and disulfiram causes unpleasant symptoms if you drink.(Source 2)
  • People with OUD: buprenorphine-naloxone (Suboxone) and extended-release naltrexone (Vivitrol) are FDA-approved options.(Source 3)
  • People with other substance use disorders, such as cannabis or stimulant use disorder, who want to ask about available off-label options

What to expect at I Care Psych

  • Any of our providers can prescribe MAT: Dr. Neel Patel, M.D., double board certified in General Adult Psychiatry and Addiction Psychiatry; Maram Yousef, PA-C; Rita Behr, PMHNP; and Khushbu Dalsania, PMHNP.
  • Visits in our Land O Lakes office or by telehealth.
  • A plan built around your needs. Counseling is offered here at I Care Psych as part of your care.
  • Book online through healow or call 813-606-5668. Please check your insurance coverage with our office.

Good to know

  • Before starting naltrexone for OUD, you need to be opioid-free for a period of time to avoid withdrawal.(Source 5)
  • Disulfiram does not reduce cravings or treat withdrawal.(Source 6)
  • Talk with your provider before stopping any of these medications, and lock medications kept at home away from children.(Source 1)

Dual Diagnosis

About 46% of adults with a substance use disorder (SUD) also have a mental illness, and about 35% of adults with a mental illness also have an SUD. Treating co-occurring mental health and substance use disorders together leads to better health outcomes.

What it is

Dual diagnosis, also called co-occurring disorders, means you have a mental health condition and a substance use disorder at the same time.(Source 1) The two often affect each other and can share causes, such as stress or trauma.(Source 2) Research suggests it is usually better to treat both at the same time, in one coordinated plan.(Source 3)

Who it may help

  • Adults living with depression, anxiety, PTSD or bipolar disorder who also struggle with alcohol or drug use(Sources 2 and 3)
  • People who use substances to cope with stress or difficult feelings(Source 2)
  • Anyone treated for one condition who feels the other was missed

What to expect at I Care Psych

  • Adult care, in the office or by telehealth.
  • Care led by a team that includes Dr. Neel Patel, M.D., double board certified in General Adult Psychiatry and Addiction Psychiatry.
  • One assessment that looks at your mental health and substance use together.
  • Then one combined care plan that may include medication and counseling. Counseling is offered here at I Care Psych.
  • Book online through healow or call 813-606-5668. Please confirm your insurance coverage with our office.

Good to know

  • Co-occurring disorders are common. About 46% of U.S. adults with a substance use disorder also have a mental illness,(Source 4) and about 35% of adults with a mental illness also have a substance use disorder.(Source 3)
  • Symptoms of the two conditions can overlap, so a careful, complete assessment matters.(Sources 2 and 3)
  • Combining benzodiazepines (some anxiety medicines) with medicines for substance use disorders can cause serious side effects. Tell your provider everything you take.(Source 1)

Spravato (esketamine)

Spravato (esketamine) is an FDA-approved nasal spray for adults with treatment-resistant depression (TRD), and, together with an oral antidepressant, for adults with major depressive disorder (MDD) with suicidal thoughts or actions. Spravato works by targeting the NMDA receptors in the brain, helping to alleviate symptoms of severe depression. Self-administered under medical supervision at our clinic, this innovative treatment offers new hope for patients struggling with persistent and debilitating depression.

What it is

Spravato (esketamine) is a prescription nasal spray for adults. It is FDA-approved for treatment-resistant depression (TRD), alone or with an antidepressant taken by mouth.(Sources 1 and 3) With an oral antidepressant, it is also approved for depressive symptoms in adults with major depressive disorder (MDD) who have suicidal thoughts or actions.(Source 1)

Who it may help

  • Adults whose depression has not improved enough with other antidepressants(Source 1)
  • Adults with MDD and suicidal thoughts, along with an oral antidepressant. It does not replace hospital care when needed.(Source 1)

What to expect at I Care Psych

  • First, you have an evaluation visit with one of our providers, in the office or by telehealth, to see if Spravato is right for you.
  • We are a certified treatment center in the SPRAVATO REMS program. You enroll in the program before your first dose.(Source 2)
  • You give yourself the spray in our office while a provider watches. We check you, including your blood pressure, for at least 2 hours after each dose.(Sources 1 and 2)
  • Please call 813-606-5668 to check your insurance coverage.

Good to know

  • You will need someone to drive you home. Do not drive or use machinery until the next day after a restful sleep.(Source 1)
  • Common side effects include feeling disconnected (dissociation), dizziness, nausea, sleepiness and higher blood pressure. They usually go away the same day.(Source 1)
  • Spravato has a boxed warning: it can cause sleepiness, dissociation and slowed breathing, and it can be misused.(Source 1) Antidepressants may increase suicidal thoughts in people 24 and younger.(Source 1) If you have thoughts of suicide, tell your provider right away, or call or text 988.(Source 4)
